Job Description

Clinic RN / Registered Nurse

• Home Health, Home Care, Geriatric Nursing experience preferred.
• Fort Collins, CO
• Now hiring for November and December start dates!

Position Overview:

• $50/hour
• $75/hour overtime
• Monday - Friday
• No weekends or holidays!

Position Details:

• Deliver skilled nursing care and assessments in participant homes, ensuring quality and safety.
• Coordinate care plans, medications, and physician orders across the interdisciplinary team.
• Educate participants and families on treatments, medications, and self-care practices.
• Triage participant needs, manage follow-up after hospital or specialist visits, and adjust care as needed.
• Document all nursing interventions, changes in condition, and progress in electronic records.
• Support and supervise LPNs, CNAs, and home care staff to maintain consistent, person-centered care.

Qualifications:

• 3 years + RN experience
• Home Health and Geriatric Nursing experience highly preferred
• AHA BLS
• CO or multistate RN license in good standing
• Clean BG upon hire
