Primary Responsibilities (including, but not limited to):
- · Travel to clients' homes and complete all documentation for nursing assessments and observations
- · Plan evaluations pertaining to clients and caregivers
- · Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan
- · Coordinate with management on implementing and revising operational procedures related to quality client care
- · Utilize professional assessment skills to identify physical, mental, and psycho-social needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach
- · Function as a resource and mentor to peers and subordinates to maintain professional nursing standards
- · Act on client’s behalf to assure patient rights
- · Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitude and practices displayed by other staff members
- · Document any client complaints concerning a caregiver and review complaints with the management team
- · Coach all staff in proper safety procedures and consistently reinforce procedures

Qualifications:
- · Able to work as a team member
- · Able to maintain confidential information
- · Able to communicate effectively with company staff, clients, client families, and referral sources, community entities
- · Ability to effectively problem solve
- · Able to supervise (instruct, assign work, etc.) caregivers while on duty
- · Excellent verbal and communication skills, adaptable in different situations, and possess excellent client interaction skills
- · Able to multi-task and work independently
Required Education and Experience:
- · Associate Degree in Nursing or Bachelor of Science in Nursing)
- · Current, unencumbered RN license
- · CPR Certification
- · Current TB Test
- · Must maintain CEU requirements as required by the Board of Nursing
- · Valid Driver's License and reliable transportation
