We are looking for a skilled Registered Nurse (RN) to provide patient-centered care in a home setting.

Full-time and part-time positions are available.
Conduct in-home patient assessments and develop individualized care plans
Administer medications, IV therapy, wound care, and other nursing interventions
Monitor patient health status and adjust care plans as needed
Collaborate with physicians, therapists, and family members to coordinate holistic care
Educate patients and caregivers on medication use, chronic condition management, and preventative care
Ensure compliance with home care protocols and state/federal health regulations
Provide emotional support and advocacy for patients and families
Education: Associate or Bachelor’s degree in Nursing from an accredited institution
Licensure: Active, unrestricted Registered Nurse (RN) license in the state of practice
Experience: At least 1 year of clinical experience preferred; home health experience is a plus
Certifications: Current CPR/BLS certification
Strong communication, time management, and clinical decision-making skills
Reliable transportation and valid driver’s license required
Experience in home health, hospice, or community-based care
Proficiency in using EMRs
