Salary : 23.00 hourly

What You’ll Do:

As the Case Manager, you’ll be the heartbeat of our operations — working directly with individuals experiencing homelessness to help them stabilize, access safe and sustainable housing, and connect with vital supportive community resources. You will play a key role in ensuring top-quality care, client-centered care while fostering a culture rooted in compassion, respect, and growth. You will:

• Conduct assessments to identify client needs, strengths, and goals.
• Develop and implement individualized service plans focused on housing stability, employment, health, and wellness goals.
• Provide crisis intervention and assist clients in resolving barriers and promoting long-term stability.
• Advocate for clients — connecting them to resources, benefits, and housing while empowering self-sufficiency.
• Build strong relationships with community partners and referral sources.
• Ensure a safe, welcoming environment for both clients and staff.

What You Bring:

• Bachelor’s degree in Human Services (preferred) or equivalent experience.
• Experience supporting homeless populations and individuals with mental health or substance use challenges.
• Strong problem-solving skills, trauma informed, and client-centered approach.
• Excellent communication and organization skills.
• Proficiency with Microsoft Outlook, Word, Excel, and PowerPoint.
• A reliable vehicle , valid driver’s license, and willingness to travel locally.
• Bilingual skills are a plus!
